This applies to the UK. During the past 3 or 4 years, I have seen primary school children taking an A3 sized bag which is the same colour as their school jumpers with the logo on it. They are also carrying a normal school bag and sometimes a lunch box or bag.
As I don't know of anyone with a primary school aged child, I cannot ask them.

What is the teachers definition of a sack lunch?

My son is going on a field trip and his teacher sent a letter out saying "bring a sack lunch. No lunch boxes." I thought a lunch box was the same as a sack lunch. Is there a subtle difference between the two? Does she just mean throw his lunch in a bag?

Yep. Put it in a brown bag or plastic grocery bag. That way he can just throw everything away and not have to worry about carrying around a lunchbox all day.
